2026 Philly Girls Got Game 

Waiver of Liability & Emergency Medical Authorization

I give permission for my child to participate in all Philly Girls Got Game (PG3) programs and activities. I understand that participation in sports and physical activities involves inherent risks, including the risk of injury. By allowing my child to participate, I voluntarily assume these risks on my child's behalf. In consideration of my child's participation, I release and hold harmless Philly Girls Got Game (PG3), its directors, staff, coaches, volunteers, sponsors, partners, and agents from any claims or liability arising from participation in PG3 activities, except where prohibited by law. I understand that PG3 staff and volunteers are mandated reporters and are required by law to report suspected child abuse or neglect to the appropriate authorities. In the event of an emergency, I authorize PG3 to obtain emergency medical treatment for my child if I cannot be reached. This includes treatment provided by first responders, EMTs, physicians, or other licensed medical professionals. I certify that I have provided accurate medical information, including allergies, medications, and any activity restrictions, and I understand that I am responsible for all medical or dental expenses incurred on behalf of my child.

Photo & Media Release

I grant Philly Girls Got Game (PG3) permission to photograph, film, or record my child while participating in PG3 programs and activities. I understand these images, videos, or recordings may be used by PG3 for educational, promotional, fundraising, marketing, and other nonprofit purposes, including print materials, websites, social media, news media, and other digital or electronic platforms, without compensation. I also understand that PG3 may collect participant information for program evaluation and reporting purposes. Any data used for research or reporting will be presented in aggregate and will not personally identify my child unless additional permission is obtained. By signing this registration, I release PG3 from any claims related to the authorized use of my child's image, likeness, voice, or recorded participation. This authorization remains in effect unless revoked in writing, except for materials that have already been published.
